ABDM for dental clinics: ABHA, HFR, HPR and DPDP
What ABDM means for an Indian dental clinic: registering on HFR and HPR, creating or linking ABHA, sharing FHIR records with consent, and the DPDP Act 2023.

Is ABDM mandatory for a dental clinic in India?
Not today. The Ayushman Bharat Digital Mission (ABDM) is a voluntary national framework for linking health records, run by the National Health Authority. Several states and insurers are nudging clinics toward it, and some government schemes and corporate tie-ups ask for an ABDM-registered facility. The practical reason to do it early is that patients increasingly carry an ABHA number from hospitals and labs, and a clinic that can link to it looks organised.
What are the four building blocks of ABDM?
- ABHA (Ayushman Bharat Health Account): a 14-digit health ID for the patient, created with Aadhaar or a mobile number. Think of it as the patient's key to their own records.
- HFR (Health Facility Registry): the registry of clinics and hospitals. Your clinic registers once and receives a facility ID.
- HPR (Healthcare Professionals Registry): each dentist registers with their Dental Council registration number and receives a professional ID.
- HIE-CM and FHIR records: the consent-managed exchange through which a record created at your clinic is made visible to the patient and, with the patient's consent, to another provider. Records are exchanged as FHIR bundles (for example a prescription, a diagnostic report or an OP consultation note).
Step 1: register the clinic on HFR
Go to the Health Facility Registry portal, sign in with an ABHA-based login for the owner, and fill in the facility profile: name, address, ownership type, specialties (dental), working hours and the dentists who practise there. Upload the clinic registration certificate under the state Clinical Establishments Act where applicable. Approval usually takes a few working days and produces a facility ID that your software stores.
Step 2: register each dentist on HPR
Each dentist creates an HPR ID with their Dental Council of India or state council registration number, qualification and a photo. Visiting consultants register once and can be linked to several facilities, which is how a visiting endodontist appears correctly on records at four clinics.
Step 3: create or link ABHA at the front desk
At registration, ask the patient whether they already have an ABHA. If yes, link it by number or QR scan with an OTP to their registered mobile. If not, offer to create one using their Aadhaar OTP or mobile number. It takes under a minute. Record the patient's consent for creation and linking, because the DPDP Act treats this as personal data processing.
Step 4: share records with consent
When a visit is complete, your software packages the consultation note, prescription and any report as a FHIR bundle and notifies the patient's ABHA that a record is available. The patient sees it in their ABHA app. If another provider later requests the record, the patient grants or refuses consent in the app; your clinic never has to find and forward it by hand.
How does the DPDP Act 2023 fit with ABDM?
The Digital Personal Data Protection Act 2023 applies regardless of ABDM. For a dental clinic it comes down to six habits:
- Collect only what you need and tell the patient why (registration form notice in English, Tamil or Hindi).
- Record consent for WhatsApp communication, marketing and record sharing separately, with time and source.
- Restrict access by role: the front desk does not need clinical notes, the visiting consultant does not need other dentists' patients.
- Keep an audit log of who viewed or exported a record.
- Honour deletion and correction requests within a defined time, and document what was removed.
- Keep data on servers in India with encrypted backups, and have a written breach procedure.

What does ABDM cost a dental clinic, and how long does it take?
Registration on HFR and HPR is free. The real cost is time: about an hour of the owner's time for the facility profile, fifteen minutes per dentist for HPR, and a short training session for the front desk on ABHA creation and consent. Approvals usually arrive within a few working days, although state verification can take longer. The software side is where clinics spend money, because only an ABDM-integrated system can create ABHA numbers, link records and respond to consent requests. Ask your vendor whether ABDM features are included in the plan price or sold as an add-on.

What do patients ask at the front desk?
Most patients have heard of ABHA from a hospital or a vaccination drive but are unsure what it does. A short script helps: "This is your free national health ID. If you link it, your dental records from today will be visible in your ABHA app, and no one else can see them without your permission. It is optional." Patients who decline are treated exactly as before. Patients who worry about Aadhaar can create an ABHA with a mobile number instead. Record the choice either way, because the DPDP Act expects you to show the patient was informed.
What should you ask your dental software vendor?
Is the product registered as an ABDM-integrated HMIS with a sandbox-tested M1 (ABHA creation), M2 (record linking) and M3 (consent and discovery) milestone? Where is data hosted? Can roles be set per user? Is there an audit log and an export? Does WhatsApp messaging run with logged opt-ins? A vendor that can answer all five in one screen is doing the compliance for you.

Questions people ask
Does every patient need an ABHA to be treated at a dental clinic?
No. ABHA is optional for the patient. Clinics should offer it, record the patient's consent or refusal, and treat patients who decline exactly as before. In Dental Software India the ABHA field is simply left blank for patients who decline; note the refusal in the patient notes so the clinic can show the patient was informed, as the DPDP Act 2023 expects.
Can a visiting dentist be registered at more than one clinic on ABDM?
Yes. An HPR ID belongs to the professional, not the clinic, and can be linked to several HFR facilities. A visiting endodontist registers once and appears correctly at every clinic. Storing HPR IDs against each dentist and consultant in Dental Software India is planned with ABDM record linking.
Is ABDM registration the same as DPDP Act compliance?
No. ABDM is a voluntary records-exchange framework run by the National Health Authority, while the DPDP Act 2023 is a data-protection law that applies to every clinic whether or not it uses ABDM. A clinic needs consent, access control, audit logs and deletion handling for DPDP in any case.
Is ABDM registration free for dental clinics?
Yes. Registering a clinic on the Health Facility Registry and dentists on the Healthcare Professionals Registry is free. The cost lies in software that can create and link ABHA records.
